All the information you tell us in clinic is confidential. This means that we are not going to routinely discuss you with anyone else.This includes your GP and other departments within the NHS Trust. Occasionally though, we may need to discuss you with other professionals if we feel there is a risk of harm to either yourself or to someone else.
This confidentiality applies just the same if you are under 18 or under 16.
Before embarking on any examination or screening tests we will gain your agreement to do so.
If you are under the age of 16 years you can consent to medical examination, investigation and treatment if you have sufficient maturity and judgement to enable you to understand what is proposed and its implication. The more serious the medical procedure proposed, the better the grasp of the implications is required.
As a new patient, you will be asked to complete this registration form on arrival. You may wish to print this out and complete prior to your visit.
As part of our service you will be asked to complete a Family & Friends comments card each time you visit.This is an ongoing part of our work and enables us to understand how our service users feel about the care they receive. Please do complete these cards whenever you can, but if you don’t have time we will be adding this form onto our website very soon
